Just like the schematic over at fuzz central. Except Q2 is a silicon transistor & the 470k resistor is a trimpot. (470k?)

How to wire the dual reverse log 100k potentiometer

And by bypassing this 100k resistor you get a vibrato effect! Add an SPDT switch to go between "vibrato & chorus"-mode.
